How to Diagnose Patients Better - and Treat Patients Better - from Patient's Viewpoints

Wouldn't you think Healthcare Providers would Ask patients how they think they can be better diagnosed and treated better as a Patient?

Yes, Healthcare Providers have Satisfaction Surveys, but what do they do with the gutsy down-to-earth comments that might interfere with their business model or upset the Insurance Companies?  

These comments might refer to Patient Listening, Bedside Manner, Treating the Whole of Me instead of just my Symptoms.

It is my goal to write a book on "How to Diagnose Patients Better - and Treat Patients Better - from Patient's Viewpoints".  

Below is a start of my research on what Patients Think and Feel about the current ways of Diagnosing and Treating Patients.  I'm going to add my own experiences and offer up ideas on "what else might be done".


Who knows if my work will change anything?  I don't know, but I think this book should be written.  Maybe future generations will find it helpful.  Wish me luck.

How to Peer Inside the Psyche of a Patient in Diagnosis and Treatment

We now have many sci/tech tools that make it possible to better "Peer Inside the Psyche of a Patient".

The goal of this is to find out what is going on Inside that might be causing/aggravating Physical/Mental Symptoms.

Of course, the patient must be willing to undergo such Invasive/Personal inspection, but some folks might be willing to do this in hopes of feeling better.  Some people with Chronic Conditions, such as Chronic Pain, seem to go to all lengths to find relief.

This new novel approach is actually "old medicine" going back centuries, but now we have new sci/tech tools.

Essentially, "Peering Inside the Psyche" of a person involves monitoring the following in Doctor-Patient Interviewing:

• Recording and analyzing the Patient's answers to the Doctor's Questions.
• Recording and analyzing when the Patient tells their Medical History and more importantly their Life Story.
• Recording and analyzing the Patient's Body Language in Discussions.
• Recording and analyzing the Patient's Physiological Signs in the Diagnosis and/or Treatment Process.  It is especially important to record/analyze Physiological Signs (Vital Signs) that relate to Emotional Distress (Stress) and/or Pain.

All of this sounds like Polygraphy or Lie Detection, doesn't it?  Yes, the same techniques are used as well as more.

To my knowledge, no one is trying to "Peer Inside the Psyche" of a willing patient, but I think it will come.

Perhaps the purpose of this article is to raise awareness of "what could be done" for chronically ill patients.

How to Personalize the Placebo Effect, similar to Personalized Medicine - Upcoming Book

It seems reasonable and logical to me, to Personalize the Placebo Effect for each individual patient.  

Next year, I will be publishing a book describing how each Healthcare Provider, e.g. Doctor, could Enhance the Placebo Effect of the prescribed Medicine/Treatment for each Patient.  

This might lead to less dependence on medicines/treatments, with their accompanying side-effects.

This book will be more of a How-To Book for Healthcare Professionals and even Patients.

What the doctor says or does makes a huge difference in the Perceived Effectiveness of Medicines/Treatments.

To accomplish this book, I will be looking at "what's been done before" and offer up ideas on "what else could be done".  This might be a first-of-kind book on this topic, from which others could build upon.

From what I see so far by my preliminary research below, the Placebo Effect is affected by many factors including the Patient's Personality, Age, Race, Gender, Religion, Culture, Belief Systems, etc.

By developing a Placebo Maximization Profile for each Patient, the doctor could say or do things to Maximize the Perceived Effectiveness of the prescribed Medicine/Treatment.  

The doctor/nurse could also advise the patient how to continue the enhance perceived effectiveness at home.

I foresee learning about the patient by prepared patient questionnaires to generally help the doctor "learn about the patient", i.e. "find out what makes the patient tick".   

In addition to developing a Placebo Maximization Profile, this questionnaire would help the doctor and patient get along better and improve communication. generally.

An Audacious Patient-Doctor Matchmaking Tool and/or Service Coming Soon!

I don't know about you, but when I need a new doctor, all I have to go by are the doctor's credentials, specialties, etc.

But, what about the doctor's personality?  What kind of person is each doctor on this list of possibilities?

Even when I call and ask for help finding a doctor, all they repeat back are the doctor's credentials and specialties.

Some doctors have a web page and maybe a video which helps some, but not much.

Why can't we patients have a sci/tech tool that helps us match ourselves to a doctor who we can get along with - and a doctor who knows what they're doing?

A Doctor-Patient Relationship is very different from taking my car to a Mechanic to be fixed.  I need to trust the doctor and be honest with what's going on with me.  Only then, can a doctor treat me properly.  If I shut down in front of a doctor, the relationship goes nowhere.  I get discouraged and the doctor doesn't know why I don't get better.

Perhaps this article will stimulate innovation to implement such a tool.  Please see below for start materials.

For myself, I'd like the Patient-Doctor Matching Tool have these features:

• Match Patient Needs first (specialist, family doctor, etc.)
• Narrow results to those providers covered by Patient's Insurance.
• Narrow by Patient's Personality vs. Doctor's Personality
• Narrow by Patient's Satisfaction History (Something like Angie's List) - Also, consider Malpractice Claims or Complaints (Something like BBB complaints)
• Narrow by Distance from Patient to Doctor and Doctor Availability. 

I'd also like a Periodic Review of Patient-Doctor Performance, similar to Employer-Employee Performance Appraisals.   By doing so, both the doctor and patient know where they stand.  

• Does the patient need to do more to help themselves.  
• Does the doctor need to improve?
• Do both Patient and Doctor need to improve, change direction, etc.?

Doctor Match Quiz (Allegheny Health Network) seems to be the leader right now, but it isn't available in my area, at least.  In addition, I think much more could be done, but it's a start.

Also, perhaps this Doctor-Patient Matching could be done by a Service, similar to a Dating Agency or Employment Agency.
